SPNDS Enrollment Documents Acknowledgement Form – This Form acknowledges that you read all the documents above.

SPNDS Medical Info, Insurance & Emergency Care Consent Form – All information on this form is required. Please do not leave any area blank. Addresses and phone numbers must be included for everyone that is listed as a person authorized to pick up your child, or as someone to contact in an emergency. Health Insurance information and signatures at the bottom of the consent are required.

SPNDS Child information Sheet – You are your child’s first teacher, and this form is important to help us get to know a bit more about your child and the family.

SPNDS Civil Rights Compliance Form – Please read and sign and return this document with the others.

Student Health Form – All information on this form is required. Please do not leave any area blank. Addresses and phone numbers must be included for everyone listed as a person authorized to pick up your child, or as someone to contact in an emergency. SPNDS permission slips – These are important permissions; photo releases, walking field trips, Family directory information

Medication Log – complete this form if your child has an EPI Pen, inhaler,  or requires SPNDS to administer medication while your child is in care, please fill out this form and return it to the main office. We will not be able to administer any medication without this form.

SPNDS Special Care Plan form – please complete all questions.

Sunscreen Permission Form – Gives SPNDS permission to apply sunscreen to your child.
